North eastern health units are joining forces to better support stroke patients after they leave the hospital.
For three years, researchers followed over a hundred people in the northeast who have used the Stroke Community Navigator service, which helps patients and their caregivers adjust to life back in the community.
Study co-principal Phyllis Montgomery says they are seeing a significant improvement.
Montgomery says they found that those who have had a stroke can lead a normal life with navigation.
The service was launched in 2011 and has expanded from Sudbury into Timmins, Temiskaming Shores, the Soo and other northern communities.
